Family Meeting Consultation
Whether it’s a new diagnosis, a death, or a change in your loved one’s physical or mental abilities, families and caregivers sometimes need to make difficult decisions. As a medical social worker and therapist, I am experienced in facilitating these complicated conversations with patience and compassion. The family meeting consultation offers support with some of the following issues:
Creating and understanding advance care directives (living wills and Powers of Attorney)
Long term care planning
Setting up caregiving schedules with formal and informal supports
Deciding what to do with a family home (or other estate related matters) after a death
I can meet with you and your family (virtually or at your home) to navigate these complex and emotionally challenging situations and come up with solutions that work for everyone. If your family is struggling with a big change and need some direction and concrete plans, this service can help.
